New Patient Registration
Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode. In this respect please bring along proof of address such as a recent utility bill as well as your medical card.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team. If you are taking prescription medicines you will need to bring these with you and see a GP. If you're not on any medication please arrange to see the practice nurse. As well as checking blood pressure we also test your urine so please bring a sample with you to the health check.
Patients can choose to see either of the GP's however registration is with the practice rather than an individual doctor.
Medical treatment is available from the date of registration. Please contact reception for further information.
